Have you had a colonoscopy to screen for colon cancer?
Screening should begin at age fifty. African and American Indians may consider testing at age forty-five.
The procedure involves a video camera to collect and a biopsy tissue biopsy a biopsy for analysis and to remove polyps.

Diagnosing colon cancer is performed by colonoscopy to examine the inside of the colon. A scope is used to examine the inside of the colon through the rectum.

A colonoscopy with a video camera is used to take a biopsy for analysis to remove polyps and screen for cancer.
Staging is used to monitor the severity of cancer. Here are the staging criteria.

Stage 1: cancer has grown through the mucosa (colon wall) or rectum but has not spread through the mucosa or rectum.

Stage 2 Cancer has grown into the rectum but has not spread to the lymph nodes.

Stage 3: Cancer has invaded nearby lymph nodes but has not affected other body parts.

Stage 4: Cancer has spread to other organs, to include the liver or the lung.

Treatment for colon cancer involves removing the tumor by colonoscopy under anesthesia. There are several surgical techniques.

1. Endoscopic mucosal resection. This involves removing polyps by colonoscopy if the cancer is localized and completely contained with polyps in an early state. The polyps are removed during the colonoscopy.

2. Minimally invasive surgery. Polyps that can't be removed endoscopic resection.

3. Invasive surgery. Polyps that can't be removed during a colonoscopy. In that instance, a small incision through abdominal wall inserting several small incisions through the abdominal wall in order to insert instruments attached through cameras on video. This technique involves taking samples from lymph nodes in the area where the cancer is located. If the cancer is invasive, surgery is recommended.

4. If cancer has grown through the colon, this may require a partial colectomy. this procedure surgeon removes part of the colon that contains cancer, along with margin normal mass of tissue on either side of cancer. The surgeon may reconnect healthy portions of the colon. When it is not possible to reconnect the colon, an ostomy is recommended.

Another option is to surgically open the abdomen and construct an ostomy bag that is worn on the patient's lower abdomen.
In some cases, the colostomy bag may be permanent.

Lymph node removal is done to remove the nodes to test for cancer. In some instances, chemotherapy is recommended to destroy cancer cells.

After surgery, if cancer has spread to the lymph nodes, then chemotherapy is administered to relieve symptoms

Radiation therapy may be recommended to eradicate cancer cells. This is to shrink tumor cells by radiation and to cancer cells.

Drug therapy may be included. These include Evacizumab, (Avastin ), Cetuximab, (Erbux), and Panitumumab).

Targeted drugs given with chemotherapy may cause side effects.
Immunotherapy may be included in the treatment plan.
Traumatic Brain Injury TBI

You may imagine that TBI is an issue that concerns young
adults who suffer athletic injuries and soldiers who receive blows in active
duty. You don’t have to be a sportsperson to get TBI, although multiple TBI’s
will manifest as classic Alzheimer’s symptoms.
In the elderly, there is a concern of TBI as a result of falls, auto
accidents, penetrating wounds, and sports activities as well. Neurogenesis will
be slower if you are older. The most important thing is to avoid getting a TBI
again. You can be your own worst enemy

When people have TBI the traumatic injuries don’t just go
away. Long-term, if you are a younger person with TBI, you can recover fairly completely
over time. If you have good relationships with people close to you, avoid alcohol,
and take care of yourself through diet and exercise you will have a good
improvement. People tend to isolate when they experience TBI which causes a
downward spiral of depression. Having a purpose in life, many times this being
work and relationships, gives you a new perspective and drive to get up in the
morning.

Many accidents can result in head trauma and a concussion
also known as TBI. It can have either have temporary or long term effects.
Symptoms will appear immediately; other than neck and head pain, people will
experience a brief loss of consciousness, nausea, dizziness, sleeplessness or
oversleep, loss of balance, and a range of emotions such as anger, guilt
remorse, and self-pity. More severe symptoms may be inability to awaken, clear
fluids draining from nose or ears, weakness, numbness of toes, slurred speech
or a coma. On the most severe end of the spectrum - suicide ideation, altered
consciousness, brain death, seizures, and hydrocephalus (water on the brain).

Several alternative treatments that are not as conventional
may help recovery from TBI. Smoking marijuana decreases brain inflammation according
to the latest psychiatric literature. TBI causes brain inflammation and resulting
cognitive issues. Other than time, good self-care combined with brain-training,
perhaps sanitizing your sleep habits can prove to be the most effective way to recover
from TBI. Sometimes people will go to a combination psychiatrist and
neurologist for comprehensive treatment, addressing emotional, biological and
physical issues at the same time. The neurologist typically conducts a sleep
study. The end effect can be uninterrupted sleep with the help of a CPAP – a
little rectangular box with a cord that plugs in near the bedside and headgear
that slips over your head and tubes which slip into your nostrils. After some
practice, in most cases you will sleep really well. You get more oxygenation,
you wake up more refreshed, and these things improve cognition and recovery.

TBI in summary, is a treatable condition, even if someone
shoots you in the head. People get through it. DLBGYD - Don’t let the bastards get you

Essential Tremor (ET), is a nervous system disorder that causes involuntary rhythmic shaking, most commonly in the hands.
It is especially common when doing simple tasks such as drinking from a glass or tying your shoelaces. It can worsen over time. It is often confused with Parkinson's Disease. It can be severe in some people. It may occur at any age but is most common in those forty and older.
Those with ET develop other neurological signs and symptoms, such as an unsteady gait (ataxia.). It mainly involves the hands, legs, chin, and other body parts. Half of ET cases result from genetic mutation.
Risk factors include genetic mutation. That is, if you have a parent with a genetic mutation for ET, you have a fifty percent chance of developing the disorder yourself.

ET is more common in those ages forty or older. The condition is not life-threatening, but symptoms can worsen over time. Challenges include holding a glass without spilling, putting on makeup or shaving, difficulty with speech, and writing legibly,

Let's take a look at common eye problems that are prevalent in older adults.

Presbyopia is loss of ability to see close objects or small print. It is a slow process that occurs over time. You may not notice this change until age forty or older. It can be treated with reading glasses.

Floaters are tiny spots or specks that across the field of vision. Most people notice them in well-lit rooms or outdoors on a sunny day. They are benign, though they could indicate more severe issues, such as retinal detachment. If you notice a sudden change in your vision, schedule an appointment with your optometrist.

Dry eyes occur when tear glands cannot produce enough quality tears. The condition can be uncomfortable, causing itching or burning. Sometimes it could lead to vision loss. Those with dry eyes or can lead to tearing or irritation. The eyes try to wash out the irritation using excess tears. Consider purchasing a humidifier in your bedroom. There are over the counter eye drops, such as Thera Tears or Refresh. Special eyedrops may be used to block the tear ducts. Purchase a humidifier.

Tearing could be triggered by wind, bright light, and temperature changes. Wear your sunglasses when outside. Sometimes the condition may be due to an obstructed tear duct or a dusty environment. Protect your eyes with sunglasses.

Cataracts are cloudy areas that cover the eye lens. It can cause vision impairment. They form slowly, with redness or eye tearing. If they are large or thick, they can be surgically removed.

Glaucoma is an eye condition with increased eye pressure.
Temporal arteritis is a condition that includes severe headache. It is worstened with chewing. It may lead to vision loss over time.
Additional symptoms include low grade fever. It may be exacerbated by an impaired immune system.
These include snoring, insomnia, diabetes, and knee arthritis. BMI (body mass index) is a tool for measuring weight.

Overweight occurs when you eat more calories than you burn through daily activity. Cutting down your calory intake and increasing your activity is a solution. A sedentary lifestyle is bad for your health. An unhealthy diet and poor eating habits will result in more body fat. Instead of a coke, drink diet coke. Eat more fruits and vegetables and fruits. You can't change your genes. However, genetics affect the amount of fat you store and where it is stored. Instead of a coke, get a diet coke. Include more fruits and vegetables in your diet.

Obesity runs in families. You can't change your genes, but you can change your eating habits. Genetics affect the amount of fat you store. It is affected by family lifestyle.

If you are not physically active, you are storing calories. Inactivity leads to lower extremity arthritis.

Climb the steps rather than using the elevator.

When you go to the grocery store, read the ingredients. Be calory conscience. Read the food labels. Pay attention to the contents of fat and sugar. Practice "portion control."

Insomnia may trigger the need for snacking between meals. However, increase your activity. Climb the stairs rather than using the elevator. Snoring may be a sign of sleep apnea. You may want to contact your primary care physician to order an overnight sleep study.

Obesity can lead to shame, guilt, social isolation, disability, and lower cognition.

Walking, swimming, and bicycling, will burn calories and improve your balance and sleep quality. Limit your snacking between meals.

A ketogenic diet lowers brain inflammation and promotes cognition. Consider purchasing "The Ketogenic Diet Cookbook," Amanda C. Hughes. Easy, Whole Food Keto Recipes for Any Budget."

Keep a notebook or food diary to track how often you eat and portion control.

Weigh yourself no more than twice per week to track your progress. This will bolster your confidence and keep you on track.

Stay with your program and keep yourself busy. Don't get discouraged. Be careful during the holidays and avoid splurging.

Sundowning: Late-day confusion. The term "sundowning" refers to a state of confusion occurring in the late afternoon and spanning into the night. It can cause a variety of behaviors such as confusion, anxiety, aggression, or ignoring directions. Symptoms may lead to pacing or wandering. It is not a disease, but a group of symptoms that occur at a specific time of the day that may affect people with dementia, such as Alzheimer's disease.

The exact cause of this behavior is not known. Factors that aggravate late day confusion include fatigue, low lighting, insomnia, increased shadows, disruption of the body's internal clock, difficulty separating reality from dreams, and presence of an infection such as a urinary tract infection.

Here are tips for reducing sundowning. Try to maintain a predictable routine for bedtime, waking, meals, and activities. Plan for activities and exposure to light during the day to encourage nighttime sleepiness.

Limit daytime napping. Limit caffeine and sugar to morning hours. Avoid eating a large meal within several hours of goint to bed. Keep a night light on to reduce agitation that occurs when surroundings are dark or unfamiliar.

In the evening, try to reduce background noise and stimulating activities, including TV viewing, which may be upsetting.

In a strange or unfamiliar setting, bring familiar items, such as photographs, to create a relaxed, familiar setting. If you have a pet, the pet may lower your stress level and promote falling asleep.

Play gentle music in the evening or relaxing nature sounds such as the sound of ocean waves. Get a checkup with your care provider to determine if you might have an underlying condtion, such as a urinary tract condition or sleep apnea. The condition may worsen if sundowning develops quickly. Consider Melatonin, an over the counter medicine that induces sleep.

When sundowning occurs in a care facility, it may be due to the flurry of activity during staff shift changes or the lack of structured activities during staff shift changes.

It may be due to an injury, such as torn cartilage in the joint, as well as arthritis, or gout. Joint infection can cause knee pain. Physical therapy and knee braces can alleviate discomfort. If these treatments fail, and function is impaired, surgery is considered.

Signs and symptoms include joint swelling, stiffness, and instability to straighten the knee. You may have joint redness, and "giving out" of the joint.

There are multiple causes of knee pain. It may be due to an anterior cruciate ligament (ACL) injury, fracture, torn meniscus, knee bursitis, and patellar tendonitis. An ACL injury is tearing of the anterior cruciate ligament. The knee meniscus is a rubbery cartilage that functions as a shock absorber, It can tear due to direct trauma or twisting of the joint. It's a common injury for those who play golf. Knee fractures or ligament tears could include the kneecap, as well as trauma due to vehicle accidents. Osteoporosis increases risk for knee fracture.

Patellar tendinitis is inflammation of the knee tendons in those who jog and cycle. Bone and cartilage could break off into the knee joint. The loose body might interfere with joint movement.

Iliotibial Band Syndrome (IBS) occurs when the patella slips out of place. Distance runners are at risk.

A dislocated kneecap occurs when the patella slips out of place due to dislocation. Those with hip or foot pain should change the way they walk or run in order to avoid joint dislocation.

Degenerative knee arthritis is the most common type of joint arthritis.

Patellofemoral pain syndrome is common in joggers. It is due to instability of the patella.

Risk factors for knee pain and weakness include obesity, lack of muscle flexibility, and joint weakness. Be cautious if you take part in alpine skiing, basketball, use of rigid ski boots, and history of prior knee injury.

Here are some suggestions to lower your risk. Maintain a healthy weight, take time for conditioning your legs, and consider weight reduction. Focus on re-conditioning your hamstrings.

Finally, be "smart" regarding your exercise. You might consider
swimming, water aerobics, and walking.

Don't forget to exercise your upper extremities. Be cautious when you climb stairs. Use the handrails. Practice a healthy lifestyle, to include proper diet, and sleep hygiene. Consider tai-chi, yoga, cycling, and stationary biking.

First, talk to your doctor. Your primary care provider may evaluate you for hypo or hyperthyroidism and do other blood work. Keep an updated diary to determine your wake-sleep activity for several weeks. This can be helpful to show to your doctor during an initial visit. If the cause isn't clear, your doctor will consider an overnight sleep study at a sleep center, to rule out sleep apnea or restless legs syndrome. These are important causes of sleep disruption. During a sleep study you get hooked up to multiple sensors overnight in a comfortable bed and get checked for brain wave activity, breathing, heartbeat, eye movement, and leg twitching. More recently, simple equipment can be sent home with you to collect the data from the comfort of your own home.

Your doctor may prescribe certain medications including Eszopiclone (Lunesta), Ramelteon (Rozerem), Zaleplon (Sonata), Zolpidem (Ambien,) Edluar, Intermezzo, and Zolpimist. Work closely with your doctor as these prescription medicines could be habit-forming. Prescription sleeping pills have side effects, including daytime sleepiness and grogginess, and risk of falling. Discuss this with your provider. Melatonin, an over-the-counter supplement can be very effective and is inexpensive and readily available. Nonprescription sleeping pills containing antihistamines may cause drowsiness, confusion, memory problems, and difficulty urinating, which may be worse in older adults. Don't neglect personal medical issues, such as prostate enlargement, which could lead to excessive night-time bathroom trips.

Cognitive behavioral therapy (CBT) can be just as effective as sleep medication. CBT trains you to recognize and change beliefs that disturb sleep. CBT is a type of talk therapy (psychotherapy). You work with a mental health counselor in a structured way, attending a limited number sessions. CBT helps you become aware of inaccurate or negative thinking so you can view challenging situations more clearly and respond to them in a more effective way. CBT can be a helpful tool in treating mental health disorders, such as depression, post-traumatic stress disorder (PTSD), or an eating disorder. Not everyone who benefits from CBT has a mental health condition. It can be an effective tool for anyone to better manage stressful situations.

If you wake up in the middle of the night and your mind starts racing, get out of bed and read a magazine or calming book. You don't want to associate lying in bed with being awake and worrying. Keep your pets out of bed as they can disturb your sleep. Get blackout curtains and keep the temperature down, as the ideal place to sleep is dark, quiet, and cool. Sound machines can be purchased to filter out noises from within your home and outside, including airplanes and sirens.

It is very important to stay on a regular sleep schedule, especially so if you travel a lot for work. Exercise and activity also promote sleep. Schedule exercise at least several hours before going to bed. Do stretches and meditation in your bedroom before you go to sleep. Find ways to relax before you get in bed. A warm bath, reading, and soft music can help. Review the labels of all your over the counter medicines. You want to check the labels to determine if they have caffeine or other stimulants, such as nicotine.

We're learning more and more about blue light, which comes from electronic screens such as phones, tablets, and computer screens. Get blue light filters for your screens, download the apps to decrease blue light, or get special blue-light filtering glasses (can buy online!) Even better, just limit your exposure to electronic screens to before 6pm.

Nothing beats an afternoon cat-nap! But a word of caution, if you do take a mid-day nap, keep it to no more than fifteen or twenty minutes or it will disturb that night's sleep. Try not to nap after three pm. Avoid large meals or alcohol right before bedtime. A light snack of carbohydrates can help. Make your bedroom comfortable for sleep.

Additional relaxation treatments can include yoga, tai-chi, and self-hypnosis. Do you have a hobby to occupy your time and put you in the "zone" or "flow"? This helps decrease anxiety. Do you have a purpose in your life? Keep up your social connections. Consider volunteering at a food bank. You learn to view life with thankfulness and appreciation.

Schedule an appointment with your care provider. Bring along your medical files and get ready for the best night of sleep you've had in a while!!
Allow yourself to slow down and manage your capabilities. This is important before you flood yourself with various medications that could lead to drug interactions.

<span style="background-color: transparent; color: black; font-family: "arial"; font-size: 11pt; vertical-align: baseline; white-space: pre-wrap;">Compensatory strategies help you deal with Altzheimers. Make life easier for yourself by placing items in the same location. Make them easily visible by marking them with colorful tape. Consider using your cell phone calendar to help you remember the day of the week and the month. <span style="background-color: transparent; color: black; font-family: "arial"; font-size: 11pt; white-space: pre-wrap;">What about items you need at the grocery store? Do you have your or your to-do list? <span style="background-color: transparent; color: black; font-family: "arial"; font-size: 11pt; white-space: pre-wrap;">You can make "to do lists" for errands and appointments.
<span style="background-color: transparent; color: black; font-family: "arial"; font-size: 11pt; white-space: pre-wrap;">

<span style="background-color: transparent; color: black; font-family: "arial"; font-size: 11pt; white-space: pre-wrap;">Try making "jingles" that you sing to yourself regarding groceries. Sing the list as you write the items down on an index card. Keep <span style="background-color: transparent; color: black; font-family: "arial"; font-size: 11pt; white-space: pre-wrap;">a small notebook in your pocket to recall important information, such as appointments. If you forgot your grocery list, sing the items to yourself.
<span style="background-color: transparent; color: black; font-family: "arial"; font-size: 11pt; vertical-align: baseline; white-space: pre-wrap;">Assess your overall health. Cognition can be affected for other reasons, such as job loss, traumatic brain injury, drug abuse, PTSD (post traumatic stress disorder ), depression, and a death in the family. These conditions mimic Altzheimers.
You get B-12 from fish, cheese, dairy, meat and eggs, as well as from fortified cereals. Vitamin B-12 plays a crucial role in energy metabolism and the production of red blood cells. Because B-12 helps your body convert carbohydrates from what you eat into fuel, low levels of the vitamin can lead to fatigue. <span style="font-family: "Times New Roman", serif; font-size: 7pt;"> <span style="font-family: "Times New Roman", serif; font-size: 14pt;">Social withdrawal is typical. When a patient with schizophrenia withdraws, it is often because they believe someone may harm them. They might not be aware of the illness. Hallucinations and delusions are real to the patient. Schizophrenics have difficulty with concentration, planning, organizing, and communication. <span style="font-family: "Times New Roman", serif; font-size: 7pt;"> <span style="font-family: "Times New Roman", serif; font-size: 14pt;">Treatment goals include having few or stable symptoms. It may be due to a new mutation.

<span style="font-family: "Times New Roman", serif; font-size: 14pt;">A psychiatrist experienced in treating schizophrenia guides treatment. The team may include a psychologist, social worker, psychiatric nurse and a case manager. The team approach may be available in clinics with expertise in schizophrenia treatment. Medications are the cornerstone of treatment. Antipsychotic medications are the most commonly prescribed drugs. They control symptoms by affecting the brain neurotransmitter, dopamine.<span style="font-family: "Times New Roman", serif; font-size: 13.5pt;">

<span style="font-family: "Times New Roman", serif; font-size: 14pt;">Newer, second-generation medications are preferred. They have a lower risk of side effects. These include Chlorpromazine, Clozapine, Haloperidol, Fluphenazine, and Perphenezine.

<span style="font-family: "Times New Roman", serif; font-size: 14pt;">Older antipsychotic medicines could cause frequent and potential side effects, including possibility of a movement disorder (tardive dyskinesia) that may or may not be reversible. Once psychosis recedes, in addition to continuing medication, psychological and social interventions are crucial. <span style="font-family: "Times New Roman", serif; font-size: 13.5pt;">

<span style="font-family: "Times New Roman", serif; font-size: 14pt;">Many communities have programs to help those affected to find housing, self-help groups, and crisis intervention. With appropriate treatment, most people with schizophrenia can manage their illness. During crisis periods or times of severe symptoms, hospitalization may be necessary. If drug treatment is not effective, electroconvulsive therapy (ECT) is considered.<span style="font-family: "Times New Roman", serif; font-size: 13.5pt;">

There's a song about pretty much everything, including kidney stones, believe it or not. So there's something there for whatever you happen to be suffering, you know? Sinead O' Connor

Are you experiencing persistent, excruciating flank (lower back) pain? This may be a warning sign of kidney stones.

TESTING

Testing to verify the diagnosis includes looking for high levels of calcium or uric acid in the urine. A twenty-four hour urine collection requires drinking a lot of fluids to try and pass the stone. Imaging verification includes x-rays or CT scans.

These may show kidney stones in the urinary tract. Additional testing may include ultrasound and an intravenous pyelogram.

TREATMENT

Small stones that don't block the kidney can be treated by your family doctor. consider. You may want to schedule a urology visit or nephrologist.

Sometimes the stones are gradually passed through the ureters and into the bladder. Small stones usually pass in the urine with minimal discomfort. Drinking two to three quarts of water daily will flush out the urinary tract. In most cases, over the counter medicines, such as Motrin, Advil, or Naprosyn alleviate the pain.

Stone analysis confirms the diagnosis. Uric acid stones can be treated with allopurinol to facilitate treatment. Long-term antibiotics in small doses helps treat the Struvite stones. Cystine stones more difficult to treat. drink more fluids. Consider medication that decreases cystine in the urine. Long-term use of antibiotics in small doses.

Ultrasound may be used to pulverize the stone. This procedure lasts fourty-five to sixty minutes. Light anesthesia can facilitate the procedure.

In rare cases, large stones that do not pass are removed by a ureteroscope with a camera and the surgery is often successful. Drinking copious amounts of water will help flush out the urinary tract.

PREVENTION

Taking a thiazide diuretic may help prevent further stones. You can also reduce kidney stone risk by drinking water throughout the day. A diet that has fewer oxalate-rich foods, which include rhubarb, beets, okra, spinach, nuts, potatoes, chocolate, and black pepper is helpful as well. Calcium-rich foods may also reduce risk.

To summarize, kidney stones are not life-threatening. The pain starts with lower level pain that may intensify surprisingly quickly. It hurts like heck, similar to taking your lower lip and pulling it over your head as Bill Cosby used to say (about delivering a baby). If your doctor suspects you may have recurrent
breast cancer based on results of a mammogram or physical exam, or because of
signs and symptoms, he or she may recommend additional tests to confirm the
diagnosis. Imaging tests may include a magnetic resonance imaging (MRI),
computerized tomography (CT) scan, X-ray, bone scan or positron emission
tomography (PET) scan. Not every person needs every test.

Your doctor may recommend a biopsy procedure
to collect suspicious cells for testing. Working in a laboratory, a pathologist
examines the cells and determines the types of cells involved. A pathologist
can determine if the cancer is a recurrence of cancer or a new type of cancer.
Tests also show whether the cancer is sensitive to hormone treatment or
targeted therapy.

Treatment
for a local recurrence may
include radiation if you haven't had it before. Radiation therapy uses
high-energy beams, such as X-rays, to kill cancer cells. If you didn't have
radiation therapy for your first breast cancer, your doctor may recommend it. Chemotherapy
and hormone therapy may also be recommended. Treatment options depend on the
extent of the disease, its hormone receptor status, type of treatment you
received for your first breast cancer and overall health.

your doctor may recommend a mastectomy to remove all of your breast tissue. If
your first breast cancer was treated with a mastectomy and the cancer comes
back in the chest wall, you may have surgery to remove the new cancer along
with a margin of normal tissue.

growth-promoting effects of the hormones estrogen and progesterone may be
recommended if your cancer is hormone receptor positive. For women whose cancer
makes extra HER2 protein, the drug trastuzumab (Herceptin) can decrease the
chance of the cancer recurring.

to live longer and help relieve symptoms.

A. What is a hernia?

An inguinal (in the groin) hernia is a painful groin bulge. The lifetime risk of this condition is twenty-seven percent in men and three percent in women. It is one of the most common surgical procedures in the U.S.

It occurs when tissue, such as a portion of the intestine, protrudes through a weak spot in the lower abdominal muscles, causing a bulge, “hernia sac.” Symptoms include pain and bulging on the right or left groin at the pubic bone. Coughing, standing, and passing a bowel movement are painful.

Bulging is more prominent at the pubic bone. Symptoms are more prevalent with standing, coughing, passing a bowel movement and straining. Pain may radiate to the scrotum. A burning or aching sensation occurs at the bulge. Application of an ice pack could ease pain and swelling. This reduces swelling enough so that the hernia slides inward with direct pressure. Watchful waiting is reasonable if symptoms don’t progress.

B. Cause

Hernias can be the result of a pre-existing weak spot in the groin, known as the “superficial inguinal ring.” Low abdominal wall weakness at the inguinal canal can be present at early childhood. Hernias can develop as the result of straining during bowel movements, strenuous physical activity, chronic coughing, or sneezing. In men, the weak spot is the inguinal (groin) canal, where the spermatic cord enters the scrotum. In women, the canal carries a ligament that helps hold the uterus in place. Hernias may be inheritable. Some hernias have no apparent cause.

C. Surgery is recommended to repair the hernia if is painful or enlarging. Incarcerated hernia is when the patient can’t nudge the hernia bulge back in place. This would suggest a “strangulated hernia.” A strangulated hernia cuts off the blood flow to tissue that is trapped. It could progress to bowel obstruction, which is life-threatening and surgery is necessary.

There are two types of hernia surgery; open and laparoscopic repair. Open repair is done under local anesthesia and sedation. The incision is closed with stitches, staples or surgical glue.

Laparoscopy this minimally invasive procedure, which requires general anesthesia, the surgeon operates through several small incisions in the abdomen. Gas is used to inflate the abdomen to make the internal organs more visible. A small tube equipped with a tiny camera (laparoscope) is inserted into one incision. Guided by the camera, the surgeon inserts tiny instruments through other incisions to repair the hernia using synthetic mesh.

People who have laparoscopic repair might have less discomfort and scarring after surgery and a quicker return to normal activities. Laparoscopy allows the surgeon to avoid scar tissue from an earlier hernia repair, so it might be a good choice for people whose hernias recur after traditional hernia surgery. It also might be a good choice for people with hernias on both sides of the body (bilateral).

<span style="color: white; font-family: "cambria" , "serif"; font-size: 12.0pt;">You might consider hip replacement when you're experiencing pain that persists despite home exercises and physical therapy. Artificial hip joints have a polished metal or ceramic ball that fits into a cup liner of hard plastic. Some prostheses use a metal cup liner, which may last longer (see illustration). If function doesn’t improve, total hip arthroplasty (touching up the bone) would be the next step. The primary goals of hip surgery are to increase mobility and function.

<span style="color: white; font-family: "cambria" , "serif"; font-size: 12.0pt;">Post-operative physical therapy is crucial for improving mobility and balance. Recovery starts with simply using a wheelchair. The next step is incorporating a non-weight-bearing exercise called pool-walking. It is what it sounds like, simply walking in a pool against the water’s gentle resistance. This has a twofold effect. You increase blood flow to the hip, which speeds up healing; at the same time retraining muscles. Pool-walking also increases endorphins, hormones that give you a feel good sensation despite the pain of recovery. Gradual mobilization hastens progress and prevents blood clots.

<span style="color: white; font-family: "cambria" , "serif"; mso-ascii-theme-font: major-latin; mso-bidi-font-family: Helvetica; mso-hansi-theme-font: major-latin;">How do you know hip surgery has taken effect? When you are progressively improving and the pain is gradually diminishing, then you are on the track to recovery. Are you exercising regularly? Go on-line to find locations in your area for a fitness program. ? Weight reduction, walking, stretching, and fitness classes are prescribed.

<span style="color: white; font-family: "cambria" , "serif"; mso-ascii-theme-font: major-latin; mso-bidi-font-family: Helvetica; mso-hansi-theme-font: major-latin;">Older people may procrastinate. They may dread failure. Reassurance and motivation go a long way. We all want to get things done quickly. There are days you may not be able to summon up the energy for recovery; give yourself some grace to rest. Rest is an important part of recovery as well. Surround yourself with people in your life who are encouragers. A hopeful outlook and having a purpose in life will do a great deal in moving towards more pain-free mobility.

<span style="color: #222222; font-family: "arial" , sans-serif;">Older Drivers Driving!<span style="color: #222222; font-family: "arial" , sans-serif;">

<span style="color: #222222; font-family: "arial" , sans-serif;">Let’s look at some driving statistics. Drivers over the age of
sixty have the highest incidence of fatalities. However, ninety percent of
crashes are preventable. Thirty percent of fatal crashes occur in people
sixty-five and older. Those accidents mostly occur in intersections. I recently
experienced my first accident in forty-five years. It was scary and I don’t
want to repeat it. Yes, it occurred at an intersection and it should have been
prevented. I’m over sixty-five. It was my fault. Thank goodness, there
were no injuries. The incident compelled me to reassess my driving skills.<span style="color: #222222; font-family: "arial" , sans-serif;">

<span style="color: #222222; font-family: "arial" , sans-serif;">On the AARP (American Association of Retired Persons) website,
there is an “older driver’s review course” that I found helpful. Check it out.
The cost is $19.95 for AARP members. The entire program includes four hours of
interactive instruction that you can complete in up to sixty days. As you work
through the course, your computer will remember where you left off.
Take it at your convenience. The instruction is designed for older learners,
and it’s entertaining. Select, “Colorado” from the menu. A few things have
changed since you learned how to drive in your dad’s 1955 stick shift on rural
roads.<span style="color: #222222; font-family: "arial" , sans-serif;">

<span style="color: #222222; font-family: "arial" , sans-serif;">

<span style="color: #222222; font-family: "arial" , sans-serif;">I won’t insult your intelligence by listing all Colorado driving
rules. You know the importance of seat belts. Don’t drive and drink. Hey, wait!
We didn’t always have those laws! Your best girl used to slide across the front
seat to be close to you on dates. No more! Grand-kids must sit in the back
seat in specially designed seats.<span style="color: #222222; font-family: "arial" , sans-serif;">

<span style="color: #222222; font-family: "arial" , sans-serif;">Do you have the habit of swinging your right arm over the
passenger seat when you brake suddenly? This was to restrain and hold in place
the kid without a seatbelt in a seat that folds over to allow passengers to get
into the back seat. We have already unlearned old habits. Let's learn new
habits.<span style="color: #222222; font-family: "arial" , sans-serif;">

<span style="color: #222222; font-family: "arial" , sans-serif;">You may have some health issue to take into consideration now.
Know your prescription drugs and how they may affect your response
time. You may have passed your last eye exam, but don’t assume that you
see like an eighteen-year-old. Compensate
for a decrease in your depth and peripheral vision. When was your last vision
exam? Scan from left to right constantly. Don’t stare blankly straight
ahead.

<span style="color: #222222; font-family: "arial" , sans-serif;">

<span style="color: #222222; font-family: "arial" , sans-serif;">If night driving bothers you, be prudent and admit your
weaknesses. If glare from rain, ice, and snow confuse you, postpone the
trip. You are not as quick with your feet anymore. Reaction time has
slowed, so increase your following distance. Three seconds should pass between
when the vehicle in front of you passes a stationary object, and when you pass
it.

<span style="color: #222222; font-family: "arial" , sans-serif;">You fatigue more quickly during long trips. Don’t plan long
stretches of driving within a day. Stop for breaks and coffee. The trip you
used to take in a day may now require two days. Do I need to remind you that
alcohol consumption while driving is deadly? Even more so with older drivers.
Select a designated driver.

<span style="color: #222222; font-family: "arial" , sans-serif;">Turn down the radio as well as your noisy passengers. Drive
with your lights on during the day and night. Consider a driver's refresher
course for seniors.

<span style="color: #222222; font-family: "arial" , sans-serif;">Our cars and technologies have changed a great deal. The AARP
course can bring you up to date. Remember when an airbag was someone who
talked too much?Anti-lock brakes require a firm touch, not pumping. They
require practice. You will appreciate skidding straight instead of doing 360s.
Adjust your mirrors. Use them and know how to adjust them. Know your
“blind spots.”

<span style="color: #222222; font-family: "arial" , sans-serif;">GPS, smartphones, and the Garmin are great for finding your way.
Learn how to use them and practice while you are a passenger. Your grandkids can help you with that. Technology,
of course, is not infallible in some high density or remote areas. Keep a paper

<span style="color: #222222; font-family: "arial" , sans-serif;">Most accidents involving older drivers occur when making a left
turn. Pay attention to the green and red arrows at stoplights. No green arrow?
Do not turn left until there is a sufficient break in the traffic. Older drivers are most often stopped for failure to
yield the right-of-way. Know who must yield and who can go.

<span style="color: #222222; font-family: "arial" , sans-serif;">In our lifetimes, we have learned to drive in five lanes of
traffic in each direction and intersections stacked four bridges high, turning
right to turn left. Wow! All of this can be confusing. If this bothers you,
avoid congested roads during rush hours. Practice driving during the slow part
of the day with a more experienced driver. Otherwise, fly to Los Angeles and
don’t drive. Two sets of eyes are better than one if you are not arguing with
the other set of eyes. Consider stopping at a rest area. Pull over, get out,
stretch, settle your differences, and do some pushups. The AARP website
contains information on rules of the road, sign interpretation, crash
reporting, and vehicle maintenance. Check it out.

However, it can be deadly for children under two to five years of age. Adult flu risk factors include age over sixty five, residents who live in care centers, and those with as asthma, heart disease, obesity, kidney disease, and diabetes.
Flu symptoms include fever over 100 degrees with muscle aches. It can be transmitted from one person to another by coughing, sneezing, or handing an object to another person who has the flu. Seasonal influenza targets young children and older adults. People with weakened immune systems and those who live with other residents, such as nursing homes, are more likely get the flu. .Antibodies against past flu sickness provides no protection against the current flu virus.

Flu risk factors include having had cancer treatment, anti-rejection drugs for organ transplant, and HIV/AIDS infection. The flu virus weakens the immune system. It increases risk for those with asthma, diabetes, and heart problems. Prescription medications that shorten the clinical course include Oseltamivir (Tamiflu), and Zanamivir (Relenza), both administered by inhaler. These medicines should not be prescribed for those who have underlying COPD (Chronic Obstructive Pulmonary Disease) or asthma. Symptoms can be alleviated by drinking water, juice, and warm soups. These remedies prevent dehydration. Increasing sleep time bolsters the immune system. Tylenol or Ibuprofen alleviate muscle aches.
The Centers for Disease Control and Prevention recommends annual flu vaccination for everyone over the age of six months. Frequent hand-washing lowers flu risk. Alcohol-based hand sanitizers prevent spread of flu sickness.
Sources of flu sickness including child care centers, schools, auditoriums and public transportation facilities. Antiviral medicine side effects include nausea and vomiting. These symptoms can be lessened if the medicine is taken with food.
Each year's seasonal flu vaccine contains protection from the three or four influenza viruses that are expected to be the most common during that year's flu season. The vaccine is available as an injection or nasal spray.
The flu vaccine isn’t one hundred percent effective. Thorough and frequent hand-washing is an effective way to prevent infection. Use alcohol-based hand sanitizers if soap and water aren't available. Cover your mouth and nose when you sneeze or cough. To avoid contaminating your hands, cough or sneeze into a tissue or into the inner crook of your elbow. Flu spreads easily wherever people congregate, including child care centers, schools, office buildings, auditoriums and public transportation areas. If you’re sick, stay home for at least twenty four hours or until symptoms resolve. Drink plenty of liquids to prevent dehydration.
This will lessen your chance of infecting others. Getting more sleep can bolster your immune system. Use over-the-counter pain relievers, such as acetaminophen (Tylenol), or ibuprofen (Advil, Motrin) to combat the achiness associated with influenza.

Be careful not to take too much of Tylenol or Advil. Overuse of these medicines could cause a rare side effect, Reye’s Syndrome.
Those affected have early morning awakening that leads to poor concentration and irritability. It hampers job performance and slows reaction time.
Your doctor might have you complete a questionnaire to determine your sleep-wake pattern and daytime sleepiness. You may be asked to keep a sleep diary.
• How often do you have insomnia and when did it begin?
• How long does it take you to fall asleep?
• How many hours do you sleep?
• Do you snore or wake up choking for breath?
• How often do you awaken at night? How long does it take you to fall back to sleep?
• Are you refreshed when you wake up?
• Are you tired during the day?
• Do you doze off or have trouble staying awake while sitting or driving?
• Where do you sleep? What's the noise level, temperature and amount of lighting?
• Stressful life events?
• Alcohol and tobacco use?
• Family history of sleep issues?
• Recent travel and time changes?
• Long work hours, shift changes, and night shift?
Keep a current list of medications, vitamins, and herbal supplements, including doses. Have your sleep partner come along for your next doctor’s visit. The partner could provide observations that verify information. Your doctor could suggest an overnight sleep study. A blood test might be appropriate to rule out hypo or hyperthyroidism. .
Parkinson’s Disease and Alzheimer’s aggravate symptoms. Ritalin, prescribed for Attention Deficit Syndrome, inhibits sleep. Coffee, tea, and “energy drinks” are detrimental. Other sleep-related disorders include Sleep Apnea and Restless Legs Syndrome. Other disorders include depression, anxiety disorder, post traumatic stress disorder, and substance abuse.
If the cause isn't clear, or you have symptoms and signs of another sleep disorder, such as sleep apnea or restless legs syndrome, referral to an overnight sleep center is recommended. This study monitors brain waves, breathing, heartbeat, brain waves, eye movement, and body movement.
Behavior therapies are recommended as the first line of treatment for those with insomnia. They are equally or more effective than sleep medicines. Cognitive behavioral therapy helps or eliminates negative thoughts and worries that inhibit sleep, such as the concept, “a bad sleep night will make me sick.”.Muscle relaxation, biofeedback and deep breathing reduce anxiety. These modalities bolster breathing, heart function, muscle tension and mood.
“Stimulus control” is a technique that limits time spent awake in bed. The bed and bedroom should only be associated with sex and sleep. Sleep restriction decreases time spend in bed, causing partial sleep deprivation. This strategy creates fatigue the next night. When sleep quality has improved, time in bed is decreased. Other treatments include remaining awake. Also called “paradoxical intention,” this method reduces worry and anxiety about being able to sleep by getting in bed and trying to stay awake, rather than expecting to fall asleep.
If you fall asleep too early and awaken too early, you can use light to push back your internal clock. Go outside during times of the year when it's light outside in the evenings. Consider purchasing a medical-grade light box kept in your bedroom.
Prescription sleeping medicines, such as zolpidem (Ambien), zopiclone (Lunesta), zaleplon (Sonata) or ramelteon (Rozerem) can induce sleep. Physicians might not recommend “prescription sleeping pills” for more than a few weeks.
Over the counter nonprescription sleeping medicines contain antihistamines that cause drowsiness. These reduce sleep quality. Side effects include daytime sleepiness, urinary retention, dry mouth and confusion.
Antihistamines worsen urinary problems, causing urinary urgency during the night. Additional options include Melatonin. This over-the-counter (OTC) supplement could reduce insomnia.
Valerian is a dietary supplement sold as a sleep aid. It has a sedating effect, but hasn't been well studied. It is released into the bloodstream at dusk and tapers off in the morning. You might want to discuss it with your doctor. Those who take it at high doses or long term may develop liver inflammation (hepatitis). When it's time to stop, it should be tapered to prevent withdrawal symptoms.
Stick to a sleep schedule. Keep your bedtime and wake time consistent, including weekends.
Avoid large meals before bed. A light snack is fine. Drink less before bedtime so that you won't have to urinate as often.

Don’t work or eat in bed. Avoid TV, computers, video games, smart phones or other gadgets before bedtime.

The TV and an i-Pad may interfere with your sleep cycle.

Close the bedroom door or create a subtle background noise, such as a fan, to dampen noise sources. You might want to silence your cell phone and bedroom clocks.

A warm bath or massage can prepare you for sleep. Create a relaxing bedtime ritual, such as reading, soft music, breathing exercises, yoga or prayer. Avoid trying hard to sleep. The more you try, the more awake you’ll become.

Read in another room till you’re drowsy, then go to bed to sleep. If you can't sleep after twenty minutes, do something relaxing, such as reading. Try again.

Yoga and Tai-Chi help with sleep quality. Start slow and work with a coach or instructor who listens to your needs and limitations.
“Mindful Meditation” along with conventional treatment can improve sleep. Research suggests that regularly practicing meditation reduces stress and lowers blood pressure.
